The end of Cyberpunk 2077's main story is slowly approaching, with V's physical and mental health deteriorating. It's clear he won't last much longer.

The “Tapeworm” main mission is staged, automatically triggering a dialogue with Johnny Silverhand after completing one of the following main quests: “Automatic Love”, “Transmission”, “Life During Wartime” and finally, “Search and Destroy". So you can't just finish it directly. It's mostly a series of dialogues, and it's better to choose the right answers to improve your affinity with Johnny.
The dialogues can also take place in different locations, depending on the order in which you completed these missions.
Take the time to choose the blue answers, in order to explore the options with Johnny. Affinity gains based on your answers, to unlocking the secret ending, aren't always very clear, so it's best to avoid being hostile or taking the sigils directly. Here are some of the answers to give:
- First dialogue: "I asked you a question: what do you want?"
- Second dialogue: "Arasaka's lepers again?" then "Why are we talking about this?"

Third dialogue: "[Smoking.] You might not be wrong."
After the dialogues, you will pass out before regaining consciousness in a deserted hotel in Pacifica.
Follow Johnny to the room indicated, then search the hideout to get the dog tags and Johnny's Tank Top , an iconic piece of armor.
A new dialogue then begins with Johnny. Explore the blue answers again, and choose to accept his demands and his plan, such as "Yes." when he asks if you would take a bullet for him, "It works." "So I tell Snape everything?" and " Okay, I'll leave the controls to you. "
After that, exit the apartment to finish the quest and start the final act of the game, with the various possible endings. Be sure to complete "Chippin'in" beforehand, to unlock additional gear and endings.
